Please find attached / see below an NHSE initiative that is being rolled out in North East London. This initiative has been successfully pilot in other areas and has proven to be effective.

The scheme should work independently of general practice but there maybe patients that raise questions / concerns to their local practice…Therefore it is crucial that practices are aware of the initiative.

In addition if practices could help communicate the scheme by having the flyer visible it would help socialise the scheme.

The team have presented at the PM forum and will be presenting at Redbridge PLE. This brochure has all the information required however if there his any further information required please contact your Federation.
